The first part of the paper, Case Study Evaluation (65%), is an analysis of the disease process in general. All elements in the grading rubric must be addressed in the paper and discussion should be based on current best practices supported by citations from current literature such as systematic reviews, meta-analyses, research-based clinical practice guidelines and randomized controlled trials. The use of evidence-based practice should be evident throughout the paper by using at least 2 references for each discussion topic. The reference list should only include those references cited in the paper.

The reference list should only include those references cited in the paper. A minimum of 3 current peer-reviewed references (research articles or systematic reviews) less than 5 years old must be used to demonstrate the content reflects the most current research. Textbooks and websites (Up-to-Date, MedScape, et al) can be used but should not constitute more than 25% of resources in the reference list. The second part of the paper, Management Plan Synthesis (20%), is the application of the information from the analysis of the disease to a patient scenario. Utilization of the SOAP format is required including subjective (patient interview) and objective (physical examination) data collection required to fully evaluate the patient with the specific disease process. In addition, the student will identify 5 differential diagnoses that present in a similar fashion and discuss how each will be ruled out using evidence/ data gathered from the interview, physical examination, and diagnostic testing. Finally, the student will develop a comprehensive medical treatment plan which focuses on treatment of chronic disease, prevention of disease progression, and promotion of health and quality of life. This will include pharmacological therapy, non-pharmacological therapy, multidisciplinary referrals, and patient education. The treatment plan should be supported with rationales cited from evidence-based literature with at least 3 current peer-reviewed references (less than 5 years) to support the plan APA Format and general writing skills accounts for 15% of the grade.APA formatting should beused throughout the paper including the title page, headings, pagination, in-text citations and references. General writing skills including correct use of spelling, grammar, and punctuation will be evaluated. The paper should not exceed 15 pages excluding title page and reference pages Grading of the assignment will follow the grading rubric below. All components in the rubric must be clearly addressed in order to receive credit. (5 points) ● Objective data: Identify relevant objective data that should be collected during the complete physical examination. Identify pertinent negatives and positives. Identify diagnostic testing to be ordered and findings expected (5 points) ● Assessment: Identify the differential diagnosis for the disorder. Use assessment data to rule in or rule out possible diagnosis. ( 5 points) ● Plan: Formulate an evidence-based treatment plan to address key issues identified including therapeutics (pharmacological/ non-pharmacological), additional diagnostics, professional referrals, and patient education.
